For your convenience, Cullman Internal Medicine will file your insurance.

If we are a participating provider in your health plan, we expect you to pay your copay at the time of your visit. Your employer or your carrier should make this information available to you when you enroll. If we do not participate in your plan, or if you have commercial insurance, you are expected to pay in full at the time of service.

Patients with Medicare are expected to pay their deductible and the 20% of Medicare allowable charge. If you have BC, C+, they will pick up this 20%.

Because insurance plans differ in services covered and method of payment, it is important for you to become familiar with your own policy. In order to assist you with your insurance claims, it is essential that our office have a copy of your current insurance card, which will provide us with the information we need to better serve you.
